Activity Description:
This session will explore the transformative role of St. Vincent de Paul Charitable Pharmacy in providing free access to medications and disease state education programs for underserved populations in Texas. By sharing these experiences and strategies, the aim is to inspire pharmacists to adopt similar practices that enhance patient care and community health. Implementing these practices can result in better adherence to treatment plans, fewer hospital readmissions, and overall enhanced patient well-being.

At the completion of this activity, pharmacists and technicians will be able to:

1. Recognize the importance of collaborative practices in providing medication access and improving patient health outcomes.
2. Demonstrate effective strategies for establishing and maintaining charitable pharmacy services through partnerships with community organizations, health care providers, and funding sources.
3. Apply techniques for engaging with the community and building strong, collaborative relationships to support the pharmacy's initiatives.
4. Assess the outcomes of collaborative efforts in providing pharmacy services on patient health and community well-being, and use this data to advocate for similar initiatives in their own practices.

Texas Pharmacy Association (154) is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. Continuing education will be provided to participants who completed all course requirements.
