Summary

Activity Description:
This session will explore the expanding role of pharmacists in chronic care management (CCM) in outpatient clinics. Attendees will learn how integrating pharmacists into CCM programs can improve medication management, care coordination, and patient outcomes. The presentation will highlight billing opportunities using Medicare CCM codes, workflow integration strategies, and real-world examples of pharmacists successfully implementing CCM services. By the end of the session, participants will be equipped to advocate for pharmacist involvement in CCM, implement best practices in their own clinics, and contribute to more efficient, patient-centered care models. This session will ultimately enhance pharmacists' role in chronic disease management, reduce health care costs, and improve patient adherence and outcomes.

At the completion of this activity, pharmacists and technicians will be able to:

  1. Define chronic care management (CCM) services and explain their impact on patient care in outpatient settings.
  2. Identify billing codes and reimbursement opportunities available for pharmacists providing CCM services.
  3. Describe best practices for integrating pharmacists into CCM teams within outpatient clinics.
  4. Discuss collaborative practice agreements (CPAs) and care models that support pharmacist-driven CCM interventions.

Texas Pharmacy Association (154) is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. Continuing education will be provided to participants who completed all course requirements.
