Summary

Activity Description:
This program intends to educate pharmacists in all practice settings to adopt medication safety best practices. It will detail how to approach medication errors from a system-level and implement changes that will prevent future errors from occurring. Attendees will also be educated on the differences between human error, at-risk behavior, and reckless behavior and the suggested management techniques for each behavior type.

At the conclusion of this program, the participating pharmacist or technician will be able to:
1. Define medication safety and the need for robust medication safety programs in all pharmacy settings.
2. List the Institute for Safe Medication Practices (ISMP) Targeted Medication Safety Best Practices for Community Pharmacies and Hospitals.
3. Apply ISMP’s Hierarchy of Effectiveness of Risk Reduction Strategies by creating solutions to
medication event case studies.
4. Explain the differences between human error, at-risk behavior, and reckless behavior and the suggested behavioral management techniques.


Texas Pharmacy Association (154) is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. Continuing education will be provided to participants who completed all course requirements.
