Activity Description:
As value-based care continues to transform healthcare delivery, community pharmacies are uniquely positioned to contribute meaningfully to population health and quality outcomes. This session explores how independent and chain pharmacies can align with Accountable Care Organizations (ACOs) to provide reimbursable clinical services beyond traditional dispensing. Attendees will gain insights into the structure and goals of ACOs, the pharmacy’s role in closing care gaps, and the practical steps to initiate, negotiate, and sustain successful partnerships. Real-world examples will illustrate how pharmacists can deliver measurable impact and secure new revenue streams through collaborative care models.

At the completion of this activity, pharmacists and technicians will be able to:

  1. Define the structure and purpose of an Accountable Care Organization (ACO) and explain how pharmacy services contribute value.
  2. Identify strategies to evaluate and approach potential ACO or health system partners.
  3. Describe actionable methods for integrating community pharmacy services into ACO gap closure initiatives.
  4. Outline the key components of a successful partnership between a community pharmacy and an ACO.

Texas Pharmacy Association (154) is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. Continuing education will be provided to participants who completed all course requirements.
