Activity Description:
This presentation will explore the implementation of age-friendly care in both long-term care (LTC) and home-concierge pharmacy consulting settings, although the material is applicable to a wide variety of practice settings. Attendees will learn how to apply the 4Ms framework (What Matters, Medication, Mentation, and Mobility) to improve patient care, while addressing common challenges like polypharmacy and deprescribing in older adults. The presentation will also cover key regulatory and legal considerations, including collaborative practice agreements and patient privacy, to ensure compliance and optimize outcomes in both LTC and home-based environments. Additionally, this session will highlight the critical role of pharmacists in collaborative, interprofessional teams, offering strategies for effective communication and care coordination.

At the completion of this activity, pharmacists and technicians will be able to:

1. Describe the core principles of age-friendly care and how the 4Ms framework is applied long-term care and home-concierge pharmacy settings.
2. Identify strategies for managing polypharmacy and deprescribing in older adults.
3. Review the regulatory and legal considerations involved in providing pharmacy consulting services in LTC and home settings.
