”ACPE Texas Pharmacy Association (154) is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. Continuing education will be provided to participants who completed all course requirements.

Description: Value-based care models are evolving rapidly. The pharmacist’s role in these models is not well established or measurements clearly defined or understood as it relates to pharmacists’ patient care services. A collaborative approach to identify and develop a system for referral between pharmacists and other health care providers and between pharmacists themselves needs to be defined to ensure patient and health care team member access the pharmacists who can help address the needs of the patient, resolve issues and recommend action plans for patient management. Under value-based care models, various team members can play a vital role in meeting established measures that can lead to improved patient outcomes, cost savings, and shared savings. At the completion of this activity, pharmacists and technicians will be able to: 1. Discuss emerging opportunities for pharmacists in value-based care models. 2. Explain the factors that may influence reimbursement in value-based care. 3. Discuss examples of Alternative Payment Models (APM) and how pharmacists are participating in these models. 4. Assess potential payment model reform options and how pharmacists may be affected.

This lesson is an application-based CE activity and is targeted to pharmacists and technicians. This program has been approved for 1.5 contact hour of continuing education credit (0.15 CEUs).
