Summary

Medical providers are facing increasing pressure to address acute/episodic care, yet there is an increasing workload to manage patients with chronic conditions due to payment being tied to outcomes and quality metrics. With the decreased bandwidth of providers and limited resources, patients might wait months for an appointment with only enough time to address their chief complaint, leaving most minor and chronic conditions unaddressed.

This is the perfect storm for pharmacists. Pharmacists already have the training and skills to be great partners for medical providers in the new value-based healthcare ecosystem. Join Dr. Amina Abubakar as she shares her journey in making the dream of pharmacist-physician collaboration a reality.

At the completion of this activity, pharmacists and technicians will be able to:
1. Define the healthcare landscape of value-based care and identify how to address the quadruple aim through collaborative clinical services.
2. Identify how pharmacists can articulate their skill set to other members of the healthcare team.
3. Identify business opportunities to build a sustainable collaborative model.
