Closed loop medication administration achieved by infusion device communication with the electronic medical record (EMR) is a key next step in improving patient safety by minimizing IV medication programming errors. Medication errors have a significant impact on patients and health care costs, so many hospitals are exploring and implementing infusion pump functionalities such as auto-programming, auto-verification of manual programming, and auto-documentation within the EMR. The goal of this technology is to create more meaningful and real-time alerts to enhance patient safety, and pharmacists are an integral to making pump communication with the EMR a success. The goal of this presentation is to equip the pharmacist with the necessary knowledge to integrate these systems.

Learning Objectives for Pharmacists
At the completion of this activity, the participant will be able to:
1. Describe the key elements of drug library configurations that may need modification to meet EMR integration needs.
2. Formulate a simplified list of available intravenous medication concentrations at your institution.
3. Inspect, evaluate, and align dose and infusion duration limits between the EMR and infusion pumps.
4. Develop methods to overcome the barriers of weight-based and non-weight based dosing within the same drug library and EMR.
5. Evaluate compliance with bedside barcode scanning prior to integration.
6. Evaluate and mitigate safety risks, such as confirmation bias, associated with new infusion procedures.
7. Establish a plan for aligning system updates in response to formulary changes/new medications.
