Summary

Transitioning a patient from the hospital to the home is a critical time to ensure they understand their medication therapy to prevent adverse events. The pharmacist’s role in the improvement of care coordination and communication during care transitions reduces patient and health system costs, improves patient health outcomes, and reduces preventable medication-related events and readmissions. This session will outline the implementation of a transitions of care (TOC) service, identify barriers and opportunities for pharmacist involvement in TOC services, explain how to use pharmacy technicians’ and student pharmacists’ roles in TOC services, teach you how to apply methods and tools to improve patient handoffs across health care settings, and introduce methods to reduce costs to the health care system and generate revenue with TOC services.

At the completion of this activity, the pharmacists will be able to:
1. Outline the implementation of a transitions of care (TOC) service.
2. Identify barriers and opportunities for pharmacist involvement in TOC services.
3. Use pharmacy technicians’ and student pharmacists’ roles in TOC services.
4. Apply methods and tools to improve patient handoffs across health care settings.
5. Apply methods to reduce costs to the health care system and generate revenue with TOC services.

”ACPETexas Pharmacy Association (154) is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education. Continuing education will be provided to participants who completed all course requirements. This lesson is an application-based CE activity and is targeted to pharmacists.
